If chosen for the scholarship, send an email to Diana McWilliams, Diana.McWilliams@mountnittany.org, tell her you are a CMAS student at Penn State chosen for the de Lissovoy scholarship. 

The below forms will need completed and sent to Diana prior to starting along with the following background checks:

FBI BACKGROUND CHECK (approx. $23.00)

Fingerprinting Info for FBI Clearance (this can take 2-4 weeks for results)

To obtain fingerprints, you must first register PRIOR to actually going to a physical location to obtain fingerprinting.

1.     Go to the following site: https://uenroll.identogo.com/

2.     Employees: Enter Code 1KG756 (this will take you to the form for DHS – Employee > = 14 years contact with children)

Volunteers: Enter Code 1KG6ZJ (this will take you to the form for Pennsylvania DHS - Volunteer)

3.     Select Schedule or Manage Appointment

4.     Complete all sections on the form

5.     Submit

6.     You will receive an email confirming your registration and confirmation of appointment time and location.

7.     Report to the IdentoGO location identified in the confirmation letter. IMPORTANT!

Do not forget to bring the required documents (as outlined in the confirmation letter) to confirm your identity.

8.     Payment is required at the appointment. There is no option to pay ahead of time. The options include Business Check, Money Order, and Credit Card.

For students taking credit or for employment, please select the option  SUBMIT A NEW RECORD CHECK and complete the required info.

This is free for volunteers. Select NEW RECORD CHECK (volunteers only) and complete the required info. This will give you a certificate to print immediately after you enter the required info.
